Strangulated Obturator Hernia

Abstract
Obturator hernia is very rare but important hernia because of high rate of strangulation leading to lethal consequences. It is very infrequent that it has been correctly diagnosed prior to laparotomy, and that there has been none of a case of obturator hernia reported in Korea yet. Here, a case of strangulated obturator hernia ia presented.
The patient was 39 year-old emaciated man and was aidmitted with the symptoms of intestinal obstruction. He had history of the repair of left inguinal hernia 20 years prior to admission and has been suffering from pulmonary tuberculosis since 2 years ago. On admission, he had signs of mechanical intestinal obstruction and typical "Howship-Romberg" sign in left side.
On exploration, there was strangulated Richter¢¥s type hernia in left obturator canal, the herniated viscus being the ileum about 50§¯ proximal to the ileocecal junction. A 10§¯ segment of the ileum was resected and restored the continuity by end-to-end anastomosis. The hernia sac was resected and the defect was sutured with silk for simple closure, through the left lower paramedian incision which was made for laparotomy. Through this incision it was not unsatifactory to reach in retropubic area.
Along with a case report, the history, anatomy, mechaism, symptomatology and treatment of obturator hernia were reviewed.
